Requirements:
1. Individual Application Book Land Crew with the respective signature and stamp of legal representative.
2. Photocopy of identity card (Venezuelans and foreign residents).
3. If a crew member is a resident alien must attach a photocopy of their passport stamped displayed respective current visa.
4. Certificate of a laboratory to determine the blood group.
5. Two (02) passport size photographs in background blue.
6. A (01) copies of the qualification certificate if that is transporting goods or the original license to provide service where it is carrying passengers, provided by the Ministry of Infrastructure and the Ministry of Transportation in force with Colombia Annexes.
7. Commercial Registries.
8. Fiscal stamps (1.3 tax units)
